Do you take insurance?

I take United Healthcare, Anthem/BCBS and Beacon.

 

How do I set up an initial appointment?

You can call my office at (970) 315-7724, or you can fill out the contact form located under the “Contact Me” page. I’ll call you back within one business day.

Do you offer online appointments?

Yes, I offer both in-person and online appointments.

I conduct online appointments with real-time audio/visual sessions through a HIPAA-compliant platform for tele-health visits.

How long are the sessions and what are your fees?
All initial appointments are 90 minutes and are $275 (self pay fee). Follow-up appointments for established clients are 30 minutes and are $100 (self pay fee).
What is your cancellation policy?
If you need to cancel, please give at least 24-hours’ notice. If you do not do so, I reserve the right to charge you an office visit fee (either $275 for an initial appointment or $100 for a follow-up, whichever is scheduled).
Insurance does NOT cover a late cancellation and it does NOT cover a missed appointment. Therefore, I reserve the right to charge you an office visit fee (either $275 for an initial appointment or $100 for a follow-up, whichever is scheduled).
Do you work with a particular age group?

I work with ages 16-60 years old.

What is your professional training and experience?

I am a board-certified Psychiatric Mental Health Nurse Practitioner. I received my master’s degree in Nursing from Ohio State University. I have been working in the health care field since 2013.

Is there plenty of parking?
Yes, there are plenty of parking spaces, and you do NOT have to pay for parking.

What are your hours?

I’m available during business hours, Monday through Friday, from 8:00 am to 5:30 pm.
